Qualification

We’re excited to learn more about your company’s qualifications and explore potential opportunities together. Please review and complete the Qualification Form below and upload all required documents. This will help us get to know your expertise and ensure we’re aligned for future projects.
If you would like to contract more than $50,000 in aggregate work with Haydon at any given time you will need to complete all qualification data below. If your company would like to stay below that threshold, please attach a letter stating such in the sections requiring your uploaded financial statements. All other documents will still be required.

Minimum Insurance Requirements
Review our insurance requirements before beginning work on any Haydon project. These requirements serve as minimums only, specific projects may require additional coverages and/or higher limits which will be communicated on a project-by-project basis.
For full information and requirements, please refer to the Master General Conditions and any project-specific requirements outlined in the subcontract.
Prior to the commencement of work on any Haydon project we must have your Certificate of Insurance (“COI”) on file as follows:
- Reference any & all projects, or the specific project on your COI
- General Liability Coverage $1,000,000 each occurrence bodily injury & property damage; $2,000,000 General Aggregate; $2,000,000 Completed Operations, including a waiver of subrogation endorsement in favor of Owner and Contractor.
- Automobile Liability Coverage $1,000,000 combined single bodily injury & property damage each occurrence, including a waiver of subrogation endorsement in favor of Owner and Contractor.
- Workman’s Compensation $500,000 as per statute; Employer’s Liability $1,000,000 each accident; $1,000,000 each employee by disease; and $1,000,000 policy limit by disease, including a waiver of subrogation endorsement in favor of Owner and Contractor.
- Umbrella or Excess Liability $3,000,000 to extend on a follow-form basis to all liability coverage and limits as specified in Employers’ Liability, Commercial General Liability, and Business Auto.
- Professional Liability $1,000,000 per claim; and $2,000,000 general aggregate.
- Contractor’s Pollution Liability $1,000,000 per occurrence; $1,000,000 aggregate to include coverage for losses caused by pollution conditions that arise from the work.
- Additional Insured coverage naming Haydon Companies, LLC, and Owner as additional insured provided on CG 20 10 07 04 and the CG 20 37 07 04 or the approved equivalent. Contractor and Owner shall be named Additional Insured for the full limits of the policy.
- All coverages shall be primary and non-contributory and require a 30-day notice of cancellation.
